9/6/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Ben-Nita upon arrival to the home setting. Upon arrival to the home setting, Ben-Nita appeared to be in a content mood evident by her facial expressions such as a smile on the face. QMHP-C discussed with Ben-Nita and her family discharging planning as she has made progress while being in services. 2. Behavior: Ben-Nita participated in the session without incident; however, she was agitated at times as she was discussing how she was really upset with her mother about what was not in her basket. 1.Intervention: QMHP-C began the session by inquiring about Ben-Nita’s current thoughts, feelings and emotions. QMHP-C began session by retrieving update on Ben-Nita's behaviors via self-report. QMHP-C openly discussed with Ben-Nita about her relationship with her mother and if there have been any improvements in communication. QMHP-C engaged Ben-Nita in a therapeutic role play as well as a role play on expressing thoughts and feelings accordingly and being more assertive with those feelings without being disrespectful when explaining herself to her mother. QMHP-C provided family with resources to assist with transitioning of discharge planning process. 2.Intervention: QMHP-C provided Ben-Nita with examples of how to engage her mother in conversation and discussed being receptive to criticism and feedback from her mother whenever they engage in conversation with each other. QMHP-C role-played with Ben-Nita reactive and nonreactive responses as well as respecting her mother's limitations and boundaries which can be perceived as disrespectful. QMHP-C discussed her ability to follow rules and regulations set by her mother as she has been refusing to listen to directives while also educating her on ways to assist het with being able to manage her anger without displaying negative behaviors when she cannot have her way. 1.Response: Ben-Nita appeared to be receptive to QMHP-Cs’ interventions and services that were provided to her during the session. Ben-Nita struggle in sessions when confronted about her behaviors toward her mother as she appears to have mixed feelings. Ben-Nita has difficulty with express her feelings and thoughts as she hates her mother in one instance and loves her the next. 2.Response: Ben-Nita reports that she has close relational ties with her mother sometimes which depends on their week. Through role-playing Ben-Nita was able to understand reactive and non-reactive responses and learning that all feedback from her mother maybe positive/negative and that she has to take in consideration that she has to take the good with the bad.
